The study by Ashrafi et al offers valuable insights for health care professionals, policymakers, and researchers interested in understanding the financial aspects of surgical care, which is critically important since the operating room (OR) generates much of the margin for hospitals and health systems. The findings provide updated information on OR costs, charges, and revenues, which can be used to inform cost-effectiveness analyses, value-based care initiatives, and policy decisions. Having data on OR minutes as a denominator is somewhat unique and proves especially useful for benchmarking. The study also underscores the importance of using accurate financial terminology and recognizing the limitations of existing cost accounting methods. The analysis of trends and variations across hospital types can help identify areas for improvement and inform strategies to control costs and improve the value of surgical care. The study uses recent data (through 2022) and analyzes trends over time. One has to wonder as to why costs, and for that matter, charges, have increased so significantly over time. For instance while direct cost proportion remained essentially stable during the time interval, costs per minute grew faster at government owned facilities compared with both for profit and not-for-profit hospitals, yet government-owned hospitals had lower mean charges per minute. Is this due to government ownership itself causing higher costs or whether other factors, such as patient mix and service offerings, are responsible?
